Direct Primary Care (DPC) & insurance

Direct Primary Care is a membership-based model where your primary care is covered by a simple, predictable monthly fee. Instead of billing insurance for every visit or service, we work directly with you. This gives you longer visits, easier access to your provider, and a more personalized, relationship-centered approach.

Traditional primary care is often limited by insurance-driven volume, short appointments, and long waits.
Our DPC model removes these constraints. We keep our patient panel intentionally smaller so your provider can take the time they need — whether that’s a quick check-in or a longer visit. You get direct communication, same- or next-day scheduling, and care that’s built around your life, not rushed through a schedule. -
Deciding whether to keep insurance or a health-sharing plan alongside your membership is a personal choice.
Your membership fully covers all of your primary-care needs here at our office. However, maintaining insurance or a health-sharing plan is still important because it helps protect you from the high costs of hospital stays, emergencies, specialists, surgeries, or other care outside our practice.

We do not bill insurance for primary care services. Your membership fee covers your visits, virtual care, communication, and ongoing support with our team.
Insurance can still be used for medications, imaging, labs, and specialist or hospital services when needed. -

Your membership covers comprehensive primary care with us, but services outside our office—such as imaging, specialist visits, emergency care, or hospitalizations—are not included and would be handled separately.
In-house labs are offered at transparent, cash-friendly prices (no insurance billing or surprise charges). For medications and any outside labs or imaging, we’ll walk you through your options and help you decide whether using insurance or paying directly saves you the most money.

We partner with local labs and imaging centers for affordable, transparent pricing.
You can use your insurance or choose discounted self-pay options — whichever fits best for your needs.

You can join by submitting a request through our contact form or messaging us through Spruce. We’ll walk you through membership details, answer questions, and schedule your first visit.

Yes — as space allows. Because our patient panel is intentionally smaller, availability may be limited. If you’re unsure, reach out to us via email or phone.

Dr. Groover is accepting new patients of all ages and has immediate availability.
Johanna Wilson is also accepting new adult patients (18+), and patients who join under her are still connected to Dr. Michael Bianco’s oversight. As Dr. Bianco’s PA, Johanna Wilson provides annual visits and routine care, and patients can also see Dr. Bianco for specific visits when needed.
Dr. Bianco’s panel is full, but joining under Johanna Wilson still links your care to his medical oversight.
If you’re joining the practice and don’t have a preference, Dr. Groover is a great place to start — especially for families or patients who want the broadest scheduling flexibility.

Only Dr. Groover provides pediatric care at Bianco Primary Care. As a board-certified family physician, he sees patients of all ages — including newborns, children, and teens.
Dr. Michael Bianco and Johanna Wilson care exclusively for adults (18+).

For new patients, especially families, Dr. Groover is a great place to start. He sees newborns through adults and currently has immediate availability.
Adult patients can also choose Johanna Wilson, who is accepting new patients (18+) and works closely with Dr. Michael Bianco.
Communication & after-hours care

All communication happens through Spruce, our secure messaging app.
You can send questions, refill requests, updates, photos, or appointment needs directly to the care team. Messages are answered during business hours, with urgent concerns addressed as needed.

After-hours messages sent through Spruce will be reviewed during normal office hours and responses are typically provided within 24–48 hours.
If you have an urgent matter than cannot wait for normal business hours, we offer an urgent line through our main office number.
For emergencies, always call 911 or go to the nearest Emergency Room.

Most refills can be requested directly through Spruce. We recommend requesting refills a few days before you run out to ensure continuity.

For symptoms that are severe, sudden, or potentially serious, urgent care or the Emergency Room may be the safest option.

Visits & access

We reserve time each day specifically for urgent needs. If you contact us early through Spruce, we can typically see you the same day or the next business day.

Yes. Many concerns can be managed through secure messaging or video. If an in-person evaluation is necessary, we’ll guide you on the next steps.

Annual visits are one hour long. All other visits are 30 minutes.

Your first appointment is focused on understanding your full health history, your goals, and any current concerns. It’s an unrushed visit where your provider gets to know you and builds a care plan that fits your needs.
